2018 HFMA Region 8 Provider Scholarship Application
The scholarship program is a way for the business partners/vendors to continue to help and support educational events for providers. We appreciate each company that has assisted in funding the scholarship program.
Applications must be received by 6/15/18. Applicants who will receive a scholarship will be notified by email on or before 6/22/18. An applicant must accept or decline the scholarship within one week of notification. In the event the applicant is unable to accept the scholarship, an alternate recipient will be selected from the pool of applicants. A $100 deposit will be collected from all scholarship recipients. This deposit will be refunded to the recipient once they have attended the conference.
Only one application will be accepted per applicant.
Q: Who is eligible to apply for the scholarship?
A: Provider attendee (hospital, health system, nursing home, clinic or other direct healthcare provider of services) that would like to attend and participate in the Region 8 conference. Payers are also eligible to apply, but a limited number of spaces are available. **A provider attendee is someone who is directly employed by the provider and is not a business consultant that is hired by the provider. Q: What expenses does the scholarship cover?
A: The provider scholarship covers the registration fee for the conference plus two night’s stay in the hotel. In the event a provider has a one-way commute of over 2 ½ hours, a 3rd hotel night’s stay the night before the conference begins will be paid by Region 8. The scholarship does not include travel expenses (airfare, mileage, or meals) outside of those provided at the conference.
Q: Can multiple people from the same facility apply for the scholarship?
A: Yes, each application will be included in the overall scholarship pool.
Q: If I win a scholarship but am unable to attend, can I give it to someone else?
A: $100 deposit will be collected from all scholarship recipients. This deposit will be refunded once they have attended the conference. If a scholarship recipient is unable to attend once they have accepted the scholarship, the scholarship and deposit is only transferable to a provider attendee within the same organization. Otherwise, the deposit is non-refundable and the scholarship will be placed back into the overall scholarship “pool”.
Q: Do I need to be a current member of HFMA?
A: Yes, one of the requirements of the scholarship is that the applicant must be a member in good standing of an HFMA chapter. You must also provide your Member ID.
